NAVICARE

NAVICARE's Advocate Specialists provide biopsychosocial assessments of each patient to determine any gaps in services, and connect the patient and their family to the necessary community resources to meet their invidual needs. Based on the referral information and the completed biopsychosocial assessment, the Advocate Specialist will also help to ensure that the patient is compliant with their plan of care. Other services offered include long term care coordination and end of life decisions, such as DNRs, Living Wills, Medical Power of Attorneys, and Code Status. Patients in an unsafe or unequipped living environment are also given placement options, as well as other options to better ensure their overall health and safety. Families of our patients are provided with resources for support, counsel, and crisis intervention if needed. Our Advocate Specialists collaborate regularly with other professionals and services involved in the patient's care, ensure continued follow-up as needed, and help to modify long term goals with changes in the patient's status.
